For the health and safety of all students, New York State has
specific laws regarding physical examinations and immunizations
for all students (click here for
Health
Forms Required by Law). Health information, including verification
of immunizations, must be submitted to the school. Students must
also have s physical exam by a physician when enrolling in a school
for the first time. Physical exams by physician are also required
at other times.

What About Health Insurance Coverage?
Out of concern for the health and welfare of all of our students,
Lima Christian School requires that every student be covered by
a comprehensive injury and sickness plan, one that meets the high
cost of medical services and is accepted by local providers and
practitioners.
Included in your Parent Confirmation Packet is specific information
about the Premier Health Plan required for International Students
at LCS. When you receive the Parent Confirmation Packet, please
read carefully the letter of introduction to the program, what
is covered, and what is excluded by the policy.
The comprehensive student injury and sickness insurance plan will
cover the student for 12 months beginning August 15, 2007 to August
15, 2008. The cost for this plan for the 2007-2008 school year
is $1125 and is paid to LCS by July 20, 2007. All International
Students will be required to pay for this insurance coverage.
Students are covered anytime, anywhere, including their home country
during the coverage period.
Parents must sign and return the Student Injury and Sickness Plan:
2007 – 2008 School Year Addendum to the Parent Financial
Contract to complete the enrollment of their student in the coverage
plan. This form will be in the Parent Confirmation Packet and
will be submitted to LCS in Step 3.
